Memorial Physical Rehab Center Location: Durand, MI Date: January 2016 - April 2016 Setting: Outpatient Physical Rehabilitation Population Diagnosis: Stroke, Parkinson's, Rotator Cuff Tear, Tendinitis, Shoulder Impingement, Humerus Fracture, Wrist Fracture, Carpal Tunnel Syndrome, Trigger Finger, Humerus Fracture, Clavicle Fracture, Lateral Epicondylitis, and Unknown Congenital Condition(s). Assessments: Goniometry, MMT, Pinch/Grip measurements, Pain measurements, and D.A.S.H. Interventions: Fluidotherapy, paraffin, therapeutic ultrasound, iontophoresis, hot moist packs, splinting, therapeutic exercise (stretching and strengthening), cane exercises for AAROM, soft tissue mobilization, scapular mobilizations, glenohumeral distractions, PNF pattern exercises dumbbell weight exercises, progressive resistive exercise, pegboards, theraputty, sensory bins, shoulder arch, ADL retraining, adaptive equipment education, and occupation-based activities per client. Special Opportunities: Documentation system- complete note writing for each client (SOAP, evaluation, re-evaluation, and discharge), In service presentation to all employees about interprofessional collaboration, opportunity to work with the occupational therapy assistant, and assigned part of the caseload to lead intervention sessions. Toni and Trish House Location: Auburn, MI Date: September 2016 - December 2016 Setting: Hospice house (non-traditional placement) Population Diagnosis: COPD, Cancer (various), Renal Failure, Stroke, and Heart Disease. Many individuals presented with secondary undiagnosed psychosocial conditions due to the dying process including anxiety, depression, stress, and isolation. Assessments: COPM, OCAIRS, and IRM Interventions: Improve quality of life, pain and symptom management (positioning, relaxation, strengthening and stretching, medication management), comfort care, counseling to surviving family/friends, education, ADL management, meaningful occupation promotion (major focus on leisure and social participation), coping strategies, spirituality support, and energy conservation. Special Opportunities: Created pamphlet to educate patients/family members on dying process and role of Toni and Trish House. War Memorial Hospital Location: Sault Ste. Marie Date: May 2017 - July 2017 Setting: Outpatient and School System Population Diagnoses: A vast amount of diagnoses in the school system and in outpatient pediatrics including Autism, Down Syndrome, Developmental Delays, ADHD, and Emotional disturbances. Within in the outpatient setting and home health treated diagnoses including Multiple Sclerosis, SHA, Hip Replacement, CVA, Arthritis, Lymphedema, Respiratory failure, Knee replacement, Wounds, Epicondylitis, CRPS, CHF, Dupuytren's, Parkinson's, Cerebral Palsy, Rotator Cuff injury, Tremors, Dequervains, and Tendon laceration. Assessments: DASH, BOT-2, School Function Assessment, Sensory Profile, Purdue Pegboard, 9 hole peg test, hand dynamometer, pinch strength, MMT, and ROM. Interventions: Occupation based activities (ADLs, IADLs, school based activities, handwriting, and dressing/fasteners), Purposeful activities (practicing transfers, drawing shapes/lines, cutting, dressing/fasteners in clinic, handwriting worksheets, fine motor kits), Preparatory methods (Wilbarger brushing approach, compression/approximation/heavy work, fluidotherapy, ultrasound, iontophoresis, hot packs, splinting, theraputting, theraband, paraffin, therapeutic exercise, sensory bins, electrical stimulation) Special Opportunities: Participated in the Alert Program summer groups for pediatric sensory modulation needs. Created pediatric occupation based treatment kit based off from the Alert Program basic tenets, and presented my project to occupational therapy team. Participated in a wheelchair evaluation. Many opportunities to become proficient working with lymphedema, wound care, and within home health.
